Prestashop Custom Checkout Fields - allows the admin to add custom fields to the checkout page. Using custom fields, the admin can add extra fields to any checkout step. This module supports attachment, drop-down, text, radio button, checkbox and other types of fields.
With this extension the admin can allow the customers to view the entered input on the order confirmation page, order confirmation email, order detail page, order invoice and delivery slip.
